Prior to the appointment, the following materials are required:
- Registration Form
- New Patient Intake Form (Adult, Child & Adolescent, or Geriatric)
- Release of Information
- Anxiety and Depression Screening (adult only) - This will be sent after initial contact.
- Child Behavior Checklist (child only) - A link to this questionnaire will be e-mailed to you after the initial contact with our office
- Previous psychiatric or psychological assessments and school records, as appropriate
